New Client and Patient Registration

I authorize the veterinarians and hospital staff of Eustis Veterinary Hospital to examine, prescribe for, and treat the pets identified above. I accept financial responsibility for all charges incurred for their care. Payment in full is due when services are rendered or when the pet is released. Hospitalization or surgical treatment may require an initial payment. I understand that a $35 fee may be charged for a returned check. I agree to pay reasonable collection costs and attorney’s fees incurred in collecting an unpaid balance, to the extent permitted by law.
